Cosmos Token Claim Form

If you have a Cosmos token, you can claim your physical card by signing a message with your wallet, verifying the token(s) that you own. Once you have signed the message, you can fill out a form to claim your card(s).

Hardware wallets (Trezor/Ledger) do not currently support signing messages to verify token ownership. If your tokens are in a hardware wallet, we'll instead need to send an internal wallet transfer. This will only cost a transaction fee; no ADA will be sent outside of your wallet.
